Personal Reference Form

This form is to be filled out by pastors, employers and personal references submitted by anyone wishing to work with OBMM. Please note: We are striving to keep the standards of our work and missionaries high and accepting to those with a good Christian testimony and a design to serve the Lord. 

Please read the following questions carefully and answer frankly to the best of your knowledge. We welcome any additional comments you may have regarding the applicant. All information will be kept confidential. We trust you will give this. your immediate attention and send it to OBMM as soon as possible. Thank you for your participation for the cause of Christ.
